Key takeaways
- The SHE module has an incident reporting form, BBSO, near miss observations and a SHE dashboard.
- The dashboard shows incident-reporting compliance and incidents by category.
- Near-miss records are read on the SHE dashboard by status, facility, severity and occurrence, beside incidents and BBSO observations.
- Operators report a near miss or observation on a phone or at a terminal, and each report gets a follow-up owner.
What to capture
What do plants need to capture on the floor?

Three kinds of safety report matter most to a TPM programme. Near misses are events that could have caused harm but did not. Behaviour-based safety (BBSO) observations record safe and unsafe behaviours seen during walks (Figure 3). Incidents are events that caused harm or damage. Near misses and observations are leading indicators: they show conditions and behaviours before anyone is hurt. Capturing them is easiest when it takes a minute and when reporters see what happened to their report (Figure 2).

| Menu item | What it is for |
|---|---|
| Incident reporting form | Report an incident with its details |
| BBSO | Record behaviour-based safety observations |
| Near miss observations | Report near misses and unsafe conditions |
| SHE dashboard | See reporting compliance and incidents by category |

OSHA describes a close call, also called a near miss, as an incident that could have caused serious injury or illness but did not, and JIPM, the Japan Institute of Plant Maintenance, lists safety, health and environment among the pillars of TPM.
MaintWiz runs the safety, health and environment pillar in one AI CMMS: near misses, BBSO observations and incidents sit beside ePTW for permits and the abnormality register for unsafe conditions.

Load your near-miss log into MaintWiz in the demo: reports, BBSO and the SHE dashboard in one place.
Dashboard
What does the safety dashboard show?
The SHE dashboard presents an incident-reporting compliance gauge and incidents by category. The compliance view shows whether reporting is keeping up; the category and body-part views show where incidents concentrate.
Book a demo and bring last quarter's near-miss log; we load it and show it on the SHE dashboard.
How does it link to the rest of TPM?
Safety findings are handled alongside other abnormalities, so an unsafe condition is tracked to closure with an owner and a date. Work that needs a permit runs through MaintWiz ePTW, the electronic permit to work for the safety pillar. Safety content for operators can be kept as one point lessons.

Each near miss is logged with its exact location, reporter, severity potential and how often it could occur, and it is assigned to an owner (Figure 5). The register then shows which near misses carry the most potential for harm and who is acting on them. Because the report, the review and the action sit in one record, the safety lead can check on the dashboard each week whether people are reporting, where reports concentrate and which actions are still open.
Paper vs system
Paper cards versus a digital log
| Paper or spreadsheet | In the system | |
|---|---|---|
| Capture | Observation cards dropped in a box | A form for near misses, observations and incidents |
| Routing | Collected weekly by the safety lead | Reports are listed for the supervisor and safety lead to review |
| Follow-up | Tracked in a separate sheet | Linked to actions that can be tracked to closure |
| Reporting | Counts by hand | Dashboard by category |
| Feedback | Rarely reaches the reporter | Status is visible so reporters see the outcome |
What does the module do for safety?
The module captures incident, near miss and BBSO reports from the floor, organises them and shows them on the SHE dashboard beside your other TPM records, so the safety lead and the area owners review the same picture.
How do you set it up with your safety lead?
Categories, departments, body-part lists and form fields are matched to your plant's own definitions, usually with the safety lead, and roles decide who sees individual reports and who sees only summaries. Start with near misses in one area and add observations as the reporting habit forms (Figure 6).

Who uses it
Who uses the SHE module?
| Role | What they do in the module |
|---|---|
| Any employee or contractor | Reports a near miss or unsafe condition |
| Observer | Records behaviour-based observations on walks |
| Supervisor | Reviews reports for the area and acts on them |
| Safety lead | Reads the dashboard; leads the review and escalation |
| Plant head | Sees reporting compliance and the main categories |
How do you make reporting a habit?
- Make reporting take a minute and say what happens next.
- Tell reporters what was done, so reports are seen to matter.
- Review the dashboard weekly; ask why a category is low as well as why one is high.
- Treat low reporting as a warning, not a good result.
Safety content for operators can be kept as one point lessons.
What will you see in the demo?
- An incident, a near miss and a BBSO observation captured on a phone.
- The SHE dashboard: compliance gauge and incidents by category, department and body part.
Mistakes
Common mistakes with safety reporting tools
- Treating a low number of reports as a good result.
- Asking for too many fields, so reporting takes too long.
- No visible feedback to reporters.
- Using the dashboard to compare departments without context.
- Letting reports sit unassigned.
Can I deploy on-premises?
MaintWiz runs as software as a service (SaaS), or on-premises on your own servers on request.
How much does it cost?
Pricing is quoted on request and tailored to the modules and users you need. Book a demo and ask for a quote for your plants and users.
FAQ
Frequently asked questions
What is near miss reporting software?
Software for capturing near misses, observations and incidents on the shop floor on forms, with a dashboard that shows reporting compliance and where reports concentrate.
What is BBSO?
BBSO is behaviour-based safety observation: recording safe and unsafe behaviours seen during walks, so that trends can be discussed with the team. It is a leading indicator, collected before anyone is hurt. MaintWiz has BBSO observations beside near miss observations and an incident reporting form, with a SHE dashboard that shows them together.
What does the safety dashboard show?
An incident-reporting compliance gauge and incidents by category, so the safety lead can see whether reporting is keeping up and where incidents concentrate. Filters for facility and date narrow the view to one plant or period.
How long does it take to start?
The whole TPM roll-out takes 6 to 8 weeks with MaintWiz. Data loads through easy Excel uploads, and the implementation team gives guided project management: a project plan, milestones, set-up of the modules and training. Safety reporting can start in one area first.
Can I report a near miss from a phone?
Yes. Employees report near misses and observations on a phone where they happen, and enterprise single sign-on is supported for your users. Reports then appear on the SHE dashboard by category, department and body part.
How does it connect to permits?
Hazardous jobs run through MaintWiz ePTW, the electronic permit to work for the safety pillar, in the same AI CMMS that holds near misses, BBSO observations and incidents.
Which TPM pillar does near miss and BBSO software serve?
It serves the safety, health and environment pillar, Anzen Eisei Kankyo in Japanese. JIPM lists it among the pillars of TPM, and the module holds the incident, near miss and BBSO reports that pillar produces.
