Quality Defect Management Software with Rejection RCA

Short answer

MaintWiz quality defect management software supports Hinshitsu Hozen (quality maintenance): it records quality defects, runs rejection root-cause analysis, keeps a customer complaint register and shows a quality dashboard with rejection RCA compliance, rejections by facility, complaints by status and defect phenomena.

Key takeaways

  • The QM module has quality defects entry, rejection RCA, a QD matrix report, a customer complaint register, a dashboard and quality losses.
  • The dashboard shows rejection RCA compliance, total rejections by facility, complaints by status and rejection phenomenon defects.
  • The aim of the TPM quality pillar is to tie defects to equipment conditions and control those conditions.
  • MaintWiz is an AI CMMS built for TPM: defects, rejection RCA and complaints sit beside the equipment records. All eight pillars, 5S, OEE, Kaizen and one point lessons run in one AI CMMS, operators capture on mobile, tags become work orders, IoT signals raise work orders and ePTW covers safety. The whole roll-out takes 6 to 8 weeks.

The record

From a defect to a root cause: the record

Figure 1. The quality dashboard: rejection RCA compliance at 85.45% (47 of 55 records with an RCA), rejections by line, customer complaints by status and the rejection phenomena behind them, surface blemish first.

Most plants record rejections somewhere. Fewer connect them to what the equipment was doing. The quality maintenance pillar asks which conditions of the equipment, material and method produce good product, and how to keep them. That starts with a consistent record of each defect: what, where, when, how many and on which equipment, then an analysis of why.

MaintWiz records each rejection with its equipment, shift, phenomenon and defect class, and the rejection RCA classes the reason by 4M, so equipment-caused defects go to maintenance (Figure 2).

From a defect to a standard

The quality maintenance loop in the QM module

  1. Record the defect

  2. Run the rejection RCA

  3. Read the QD matrix report for repeat defects

  4. Set the 4M conditions as checks in the CIL or PM standard

  5. Verify with defect data

JIPM, the Japan Institute of Plant Maintenance, lists quality maintenance among the four core pillars of TPM, the pillars it describes as directly related to reducing loss in the production process.

A quality defect matrix is a table that relates each defect type to the process steps or equipment conditions that can cause it, so that the team knows which conditions to control. The QD matrix report counts rejections by phenomenon and area (Figure 3), and the free QM matrix sets the 4M condition, its value and its check frequency for each quality characteristic (Figure 4).

Dashboard

What does the quality dashboard show?

The dashboard presents the rejection RCA compliance (how many rejections have an analysis), total rejections by facility, complaint register status and the defect phenomena behind rejections. Quality leaders read it to see whether analysis keeps up with rejections and where defects concentrate.

See the RCA compliance gauge on your rejection data

In OEE, rejects and rework are the quality factor. A system that records defects against equipment lets the team read quality losses next to the availability and performance losses for the same equipment. The pillar then asks what condition of the equipment produced the defect and whether it is controlled. The rejection RCA list shows each analysis beside its equipment and defect class (Figure 5).

Figure 5. Each rejection is listed with equipment, shift, phenomenon and defect class, and the reason found is classed by 4M, machine, method, material or manpower, so equipment-caused defects go to maintenance.

How fast can we start?

The MaintWiz roll-out takes 6 to 8 weeks, with easy Excel uploads of equipment, defect types and people, and guided project management by the MaintWiz implementation team. Defect categories, RCA formats and report layouts are set up with each plant, and the QM module holds the defect, analysis and complaint records next to the other TPM records.

Complaint register

Is there customer complaint register software for plants?

Yes. MaintWiz keeps a customer complaint register with a status for each complaint, so open complaints are visible beside the rejection records for the same equipment. The QM dashboard shows complaints by status, and the QD matrix report counts the repeat defects behind them.

Rejection RCA compliance on the same dashboard shows whether analysis keeps pace with rejections, so a complaint meets a root cause that has already been recorded.

From defect record to equipment condition

  • Record the defect with its phenomenon, the equipment and the quantity.
  • Run the rejection analysis and record the root cause.
  • Update the QD matrix: which process conditions affect this defect?
  • Turn the conditions into standards that operators and maintenance can check.
  • Track the action to closure and watch whether the defect recurs.

Corrective actions are tracked like other abnormalities in the abnormality register, with an owner and a target date.

What you will see in the demo

  • A defect recorded against equipment with its phenomenon and quantity.
  • Rejection root-cause analysis and the QD matrix report.
  • Complaints tracked by status in the complaint register.
  • The QM dashboard: rejection RCA compliance, rejections by facility, complaints by status and defect phenomena.
  • Data exported to XLS and PDF.

What goes wrong

What goes wrong with defect records?

  • Free-text defect names, so the same defect appears under five labels.
  • Recording rejections but not the equipment or the condition.
  • Analysis done after the fact for only the largest rejections.
  • Closing a complaint without verifying the countermeasure.
  • Treating the dashboard percentage as the goal instead of the analysis behind it.

A short controlled list of defect phenomena, entered at the point of detection, is worth more than a long free-text field. Review the list when many defects land in 'other'.

Export, integrations and pricing

Defect, rejection and complaint data can be exported to XLS and PDF for your quality reviews, and MaintWiz can connect IoT, MES, SCADA, PLC and DCS sources. MaintWiz runs as software as a service (SaaS), or on-premises on your own servers on request.

MaintWiz supports Unicode, so defects, RCA notes and complaints can be written in Hindi, Tamil or any other script; menus and labels are in English. Pricing is quoted for the modules and users you need. Get a quote or read how TPM software is priced.

FAQ

Frequently asked questions

What is quality defect management software?

Software for recording defects, running rejection root-cause analysis, keeping a customer complaint register and reporting quality, alongside the other TPM modules. The menu covers quality defects entry, rejection RCA, the QD matrix report, complaints, a dashboard and quality losses.

What is a QD matrix?

A table that relates defect types to the process steps or equipment conditions that can cause them, used to decide which conditions to control. The QM module has a QD matrix report for this purpose.

What does the QM dashboard show?

Rejection RCA compliance, total rejections by facility, customer complaints by status and the defect phenomena behind rejections, so quality leaders can see whether analysis keeps up and where defects concentrate.

How does it work with our quality system?

Defect, rejection RCA and complaint data export to XLS and PDF for your quality reviews, and sit beside the equipment records in MaintWiz, linked to the equipment each defect came from. The QM dashboard shows rejection RCA compliance and complaints by status for those reviews.

How does it relate to OEE?

Rejects and rework are the quality factor in OEE. Defect records against the same equipment let the team read quality losses beside availability and performance.

Can we start with a spreadsheet?

Yes. The free QA matrix and rejection RCA template, the QM matrix (Hinshitsu Hozen condition table) and the customer complaint and 8D register let you start in a spreadsheet; move when rejections, analyses and complaints must be read beside equipment records.

What is rejection RCA software?

Rejection RCA software records the root-cause analysis of rejected product and reports how many rejections have an analysis. In MaintWiz the quality maintenance module holds the rejection RCA beside defects, the QD matrix report and the complaint register.

How long does it take to roll out quality defect management software?

The MaintWiz roll-out takes 6 to 8 weeks, with easy Excel uploads for equipment, defect types and people, and guided project management by the MaintWiz implementation team through set-up and training.
